First off, the mods have OK'd this, so don't worry about that.
Ladies and gentlemen, I am in need of a little help for a class for school. It's a group project, and everyone knows how much fun group projects are.
Anyways, as part of this project, we need to gather some information. The subject of our project is health care and insurance, so a logical place to start seemed to be find out, so the logical place to start was to get a baseline: how many people have insurance, how many people don't, how many get insurance through work, how many veterans make use of the VA, stuff like that. So, in my quest to find people willing to take a survey, I come here.
Now, I understand that stuff involving health care and insurance can be a touchy subject. If you would like to take the survey but don't want to post your answers, feel free to PM the answers to me or, better yet (for me when I start to compile the info), e-mail the answers to BJSchoolStuff@live.com.
Now, disclaimers:
Yes, I know this survey is pretty poorly written. It was what the group could come up with with the time we had.
Yes, I know this isn't very scientific. But all of us are doing the same survey, so I'm just going for as many response as I can. We'll see what we can do with the info later.

It seems like giving the questions in the open and letting people decide to take the survey after reading them would be likely to cause selection bias.

Anyone is allowed to cheat as much as they want whenever they get handed some dumbfuck assignment obligating them to deal with random members of the public.

Actually looking again it seems like it would make more sense to look up publicly available polling information on this subject, unless surveying is a required part of the assignment, which it doesn't seem to be.

......1)How do you feel about the quality of care you received there? It's a good quality doctor office, the sliding fee is nice because you pay according to your income if you don't have insurance. There should be more places like it (in the absence of public health insurance which would be best), but they depend on qualified doctors basically volunteering their time (as they are paid much less). For example, the main general practitioner there also works at a hospital where I am sure he makes much more money. If he wanted to have his own private doctor's office in a different location he could probably make more money, as well.

General Comments: The medical system of the U.S. is seriously screwed up. Medical care shouldn't be treated like a privilege one pays for, on the off-chance you can afford it. Plenty of people can't, and it's not because they have wasted their money. To deny needed medical care such as surgery to someone because they can't pay for it should be a crime. Health care should be a public enterprise much like the police department, fire department, etc. paid for via tax dollars. There should be a "public health insurance" option, but really it should be that everyone receives the care they need regardless of the cost, as a basic human right and right of a citizen of the United States.

Using the NPF as a free resource is actually pretty good, we're a diverse, yet similar group of people, and we're people you know, and know to be honest. Not everyone has money to print out flyers or the time to go bug a few busy college students/ business people.
Besides you think you're poor? Pyros has to buy 1 soda at the beginning of the year, then reuse it for refridgerated brew tea for the rest of the semester if I want soft drinks during class. POOOOOOOR!
When I was writing a short story for class, I had to actually reduce it's word and page count by half, cutting several important parts, just so I could afford the printing costs and still eat two meals a day! POOOOOOOOOR!
When my friends all get together and eat out, I sit outside staring at them through the window just to impress upon them how unfair it all is because I'm POOOOOOOOOOOOOOR, until they get tired of me and drag me inside and talk about how they were going to pay for me anyway.
Also, I'm an English major. POOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOO OOOOOOOR!

What kind of university doesn't provide you with so many pages per semester/year to print off in a computer lab? Is this what you are paying high-ass tuition costs for? You should bring it before the student body.
I went to a state university, they gave everybody 300 pages on their account.
EDIT: OF COURSE, IT IS BLATANT SOCIALISM TO PROVIDE PEOPLE WITH FREE PRINTING (THAT COULD EASILY BE COVERED BY A TUITION FEE SINCE THEY ALWAYS ADD LIKE 1000 DOLLARS ONTO THE TUITION WITH STUPID FEES).

My university charges by the page printed (roughly 4 cents per page), but you get about $35 worth of free printing. I don't know if that's per semester or per year. Hopefully per semester because I've got nothing but science classes in my future and they sure love giving out slides to get printed.
Though I think the implemented policy was due to Lit and Law majors printing out fucking books worth of prints, clogging the printers the damned bastards.

My capitalist swine school used to give free printing to CHASS students for CHASS related stuff, but when the school hit hard times....THEY PLANNED A MAJOR AND UNNECESSARY REBUILDING OF THE FOOD COURTS. That wasn't voted for, it was majorly voted against. And they did it anyway. People lost jobs, classes were cut, printing no longer free.
And it won't be finished until all the people who voted for/against it are gone!

I have a creative writing class! I need to make a copy of my work for the 20 students plus my teacher! I need to redo said copy for the revised version! 10 cents a copy! This means that even if I am a master of brevity and have 5-6 page short stories, I will have paid $66, if I'm an average student I'll have more pages than that and for each extra page the cost increases exponentially. I spent around $169 last semester! That's ignoring the fact that revisions tend to have more pages to them (usually) and we still have to print out responses to other classmate's stories!
If my next CW teacher doesn't allow for stories to be emailed, FIRE! FIRE UP THE WAZOO!

Yeah, tell your teacher she should definitely just have everybody put their rough drafts up on a Wikispaces (http://www.wikispaces.com/) site or Google Docs (http://docs.google.com/) or something, that's freaking insane. I have also had writing classes and they can use up your print allowance pretty damn quick, but 10 cents a copy is outrageous, especially since you are making them for the whole class multiple times for the whole semester (WTF).
Also, why the hell is your professor making you provide the copies? When I took one she just had us turn in a copy and then she got it xeroxed for the class to use during workshop. Tell your professor she should either print out the copies for the class OR you guys should move to a digital space (such as wikispaces, which is free), because she is putting you and other people I am sure into the poor house because of the university's policies. She should be aware that her students can't afford the printing costs for hard copies of stories and so should have already had some alternate plan like Google Docs at the beginning of the year. But y'know, politely suggest it and see where it gets you, show her how easy it is to upload documents and have people download them to edit and reupload, etc.
I could see printing out a final draft for the final grade but all the rough drafts and revisions could be done quite easily digitally and I'm surprised someone hasn't mentioned it to her before or that the college doesn't already have an online system for their classes to use. Like every class I ever took the professor had a site on the eCourse system with the syllabus and assignments and crap, for the creative writing class you could upload documents for others to download, discuss on a discussion board, etc.
Or if none of the writing professors are aware of these MAGICAL INTERNET THINGAMAJIGS you should transfer to another college after this semester if you can because this is sounding like some major bullshit to me. Unless you have to attend this university for your major, or if it's still cheaper than the alternatives even with the idiotic unneeded costs they seem to be heaping on you.

Yeah, ShoutWiki (http://www.shoutwiki.com/wiki/Main_Page) is free and popular for school purposes. Or if they want a free Blackboard alternative, Moodle (http://en.wikipedia.org/wiki/Moodle) is apparently quite good. Then there's always Desire2Learn (http://www.desire2learn.com/), which is used by the UW system.

Yeah, my uni just switched to Desire2Learn this year (but I'm graduated now). However it probably costs money to subscribe to so that's probably why their university isn't already using it.

Well, the class is called "Global Citizenship", so it's just the global part, I guess.
