Half Life 2: Episode 1, inside the Citadel while descending on a glass lift while huge pieces of debris are falling down. You have to use the Gravity Gun to knock them away... and I accidentally saved about a second before a huge assed piece of debris appeared and had about 1.5 seconds to try and shoot it away. No matter how many times I reloaded I just couldn't move into the right position, aim and fire in time.
Luckily it wasn't that far into the game, so starting over wasn't such a big problem.

Breath of Fire 2.
I call this The Highfort Debacle. During my very first playthrough of the game, I was leveling two of the three characters I never used (Jean and Spar with the other being Sten.) on the Giant Island using Nina's Death spell on the slimes, Chop Chop on the flys, etc... to level. I did this for about 15 mins before I got bored. Long story short I just threw Sten into my party with Spar and Jean and decided to check out Highfort in hopes of new weapon shops. I didn't realize you can't leave after you go in.I basically played the entire thing with very under leveled guys with very bad equipment. It took me two weeks of dedicated playing to get out. Hell, the solo Sten thing took me 4 days by itself due to him having not had an equipment upgrade since Windia.

Come to think of it, I think the worst time to save is about 2 hours ago, when you forget to save and go and do a ton of stuff and then fuck up.
That's more annoying for me really, because I rarely ever save at the wrong time, I just forget to before doing something time consuming or important.

Breath of Fire 2 using only emulator save states. I saved in the first bodyguard guy in the restaurant place. I hadn't even played that much, I just went there as soon as I could.

Yes, I've learned to use two save states while I play any game. One always saves in a safe place, the other I use willy nilly.
