Oh, on the PSP for SNES the savestate system has to be done in the menu. Bringing up the menu halts everything and since the emulator still isn't perfect, if the game is loading anything really huge stopping that process can cause the game to glitch and lock up.
That's a pretty bad time to save.

right before a wipe in Black Temple... oh wait
I had a C&C Red Alert: Yuri's Revenge campaign save that was right before an offensive I hadn't prepared for, so I never have the time to make a proper defense no matter how many times I load it. Each time, I had to witness the slaughtering of my troops before the might of Yuri's battalions. I mourn each and every one of them. I saved over my most recent save file in Xenosaga one time while walking into a dungeon that I hadn't the proper levels or experience to really be running around. Each random mob battle was tough as Hell, and I stood no chance at the boss. My second most recent save was like 3 hours behind or more. I stopped playing that game for a while there, especially since I prefer to play for the story than the actual grinding and battles.

I recall being little, and having that old Doom game. I had saved midair whilst falling into an acid pit. Loading meant completing the fall, and being stuck forever and dying about a minute later.
More recent screwup would be HL2. That sequence where you're up in the airducts and a whole section gives out and falls into the dreaded laser boobytrap room (it's a trap!). I had attempted to pick up an explosive barrel to move it away, and then thought to save first. Unfortunately, I dropped it and then did the save, not realizing I have inadvertently doomed myself to death by boom the moment I loaded that file.

My brother was in the habit of not fighting battles in our "shared" RPG files. This included everything from Chrono Trigger to FF-anything. I'm the type that fights every battle, by contrast, so where if I had a file I managed to keep him off of it, I'd be wiping out the next area's beasties with ease, in my shared files, I'd invariably find myself in a new area getting nearly one-hit-killed by the weakest enemies there because I'd have gone off to work for 4-5 hours and he wouldn't have fought a single battle during that time because it was too much of a hassle.

Oh I just remembered another one: Overworld save in the NES FF3. In the place where Leviathan shows up in random battles. No matter what, I'd always end up fighting the bastard, not being able to run in time and everyone would die.

First time i played FFTactics I was at about 15 hours and after one VERY difficult Boss fight it gave me the opportunity to save, and I did....little did I know it was going to take me DIRECTLY to another Boss fight that was MUCH more difficult....after a couple dozen tries I had start over from the beginning and figure out I was supposed to be at a much higher level for that fight :P Fifteen hours down the drain.
