It's just that this gate and, most likely the wall is it located in encompasses not just an asylum, but several large buildings, most likely a city. More accurately, the kind of city you'd get if the joker played Sim City. So really, I'm not sure what's happening here. Unless Joker enacted a dastardly plot to have the asylum demolished and replaced with several city blocks.
